What changed

  • Revenue was $112.7 billion, 0.4% below the $113.2 billion of the third quarter of 2025 and above the $112.0 billion of the second quarter of 2026.
  • GAAP earnings from operations were $6.6 billion, up 52.1% from $4.3 billion a year earlier. The GAAP operating margin was 5.8% against 3.8%, and the net margin was 3.6% against 2.1%.
  • GAAP diluted EPS was $4.56 against $2.59 a year earlier (+76.1%). Adjusted EPS, UnitedHealth's non-GAAP measure, was $4.69 against $2.92 (+60.6%). UnitedHealth raised its full-year 2026 outlook, which now stands at no less than $19.50 of diluted EPS and $20.50 of adjusted EPS.
  • The medical care ratio was 88.4% against 89.9% a year earlier, and the operating cost ratio was 12.8% against 13.5%.

Key facts

Medical care ratio
88.4% (89.9% a year earlier) as reported
Operating cost ratio
12.8% (13.5% a year earlier) as reported
UnitedHealthcare operating margin
3.3% (2.1% a year earlier) as reported
Optum operating margin
5.6% (3.6% a year earlier) as reported

Why it matters

UnitedHealth attributes the lower medical care ratio to medical cost management, benefit design changes and pricing. It also reports $610 million of net favorable reserve development related to 2026 dates of service, which on Blockhaven's arithmetic is about 0.7 percentage point of the quarter's $87.1 billion of premiums. UnitedHealthcare's operating margin was 3.3% against 2.1%, and Optum's was 5.6% against 3.6%.

Both segments reported lower revenue: UnitedHealthcare's was $86.3 billion against $87.1 billion, and Optum's was $66.9 billion against $69.2 billion. UnitedHealth attributes Optum Health's 10% revenue decline to fewer value-based care patients served. UnitedHealthcare served 48.3 million people against 50.1 million a year earlier.

Adjusted EPS of $4.69 is $0.13 above GAAP EPS. UnitedHealth adds back $0.41 per share of intangible amortization and removes $0.16 of net portfolio divestiture and South American impacts and $0.08 of restructuring and other items, with a $0.04 tax effect. Adjusted earnings from operations were $6.3 billion, a 5.6% margin against the 5.8% GAAP margin, because the adjustments remove net gains.

What to watch

  • Medical care ratio at the next report against 88.4% this quarter, which included $610 million of net favorable reserve development: a ratio that holds without a comparable benefit would show the improvement does not rely on reserve development.
  • Year-over-year revenue against the 0.4% decline now and 0.4% growth a quarter earlier: UnitedHealthcare membership and Optum Health revenue are the lines to follow, and a return to growth would change the picture.
  • Illustrative arithmetic only: the nine-month EPS of $17.50 (GAAP) and $18.30 (adjusted) subtracted from the full-year outlook of at least $19.50 and $20.50 leaves $2.00 and $2.20. That is not fourth-quarter guidance or a floor for it: per-share amounts do not add exactly across periods.

Data and sources

GAAP figures, ratios and the outlook come from UnitedHealth's third quarter 2026 earnings release (Form 8-K, filed Oct 13, 2026). Adjusted EPS is a company-defined non-GAAP measure from the same release. Blockhaven used no outside forecasts, so no figure here is measured against market expectations.
